  • Request non-urgent appointments
  • Ask non-urgent medical questions

How do I sign up for PxP Portal?

  • At your next appointment, provide the front office staff with your personal email address.
  • You will receive an emailinvitation to enroll.
  • Please do not share your log-on or password so that your information remains private.
